The staff therapist works in close conjunction with the Director of Rehabilitation aiding in the efficient coordination of the department programming. The therapist treats patients within the inpatient, outpatient, and home health settings according to specified prescription and protocol using any of the variety of treatment methods available. Directs patient participation in selected tasks to restore, reinforce and enhance performance, facilitates those skills and functions essential for adaptation and productivity, diminish or correct pathology and to promote and maintain health.

1. Acquires a history and evaluation for each patient treated.
a. Completes initial documentation of patient history as part of the comprehensive evaluation and within time frame established by policy and procedure.
b. Determines medical necessity and appropriate treatment plan following evaluation and documents plan in patient chart within 24 hours of evaluation.
c. Completes re-evaluations and functional reassessments within the appropriate timeframes.
d. Revises the plan of treatment following re-evaluations, functional reassessments, and as necessary.
2. Implement & modify treatment plans
a. Administers prescribed treatment for patients.
b. Recognizes and responds appropriately to changes in patient condition.
c. Maintains daily documentation for each patient.
d. Ensures safety and comfort of patients undergoing therapy.
e. Intermittently assesses patient response during therapy and determines patient ability to complete therapy session.
3. Counsels patient/family
a. Develops plans and educates patients to follow home treatment programs.
b. Documents patient program, and patient/family education in patient’s chart and on the discharge instructions.
c. Provides regular communication with patient and family to explain the plan of treatment and the goals.
4. Communicates with physicians and other health professionals
a. Communicates with the referring physician regarding patient plan of treatment, progress and recommendations.
b. Collaborates and communicates with other disciplines involved in the care of the patient.
c. Documents all case conferences / communication in the patient’s chart.
5. Supervision of the therapy assistants/ students.
a. Provides daily communication and direction to the therapy assistants relative to patient care and treatment plans.
b. Communication / supervision of the assistant is documented in the patient’s chart.
c. Serves as a resource person to assistants, clerical and other rehab staff whenever needed.
6. Aids the Director in orienting new staff / students to the department
a. Provides explanation, demonstration and supervision for orienting employees to determine ability to provide independent skill as listed on the orientation record.
a. Coordinates daily schedule with appropriate staff to ensure all area are covered.
b. Records and coordinates all patient referrals according to department policy and procedures.
c. With each patient treatment plan provides clerical staff with information regarding length of time needed for patient visits, frequency and time of day requirements
8. Appearance and Reliability
a. Observes organization and department’s dress code policy.
b. Wear identification badge while on duty.
c. Provides notification of absence from work in accordance with department and hospital policies and procedures.
d. Demonstrates timeliness in attendance and completing work-related responsibilities.
e. Attends departmental meetings/in-services as well as hospital wide in-services.
9. Housekeeping Safety and Equipment Care
a. Maintains a clean and safe environment.
b. Reports safety incidents and hazards to immediate supervisor and submit facility report and work order to ensure corrective action.
c. Promptly reports verbally and in writing all equipment malfunctions to immediate supervisor and submits a work order form.
10. Decision Making
a. Is vigilant to the day-to-day fluctuations and responds promptly to re-prioritize assignments to fit the need.
b. Performs routine assignments independently.
c. Seeks direction when indicated for unusual or non-routine duties.
d. Advises appropriate personnel of situations requiring nursing, administrative or technical expertise follow-up.
11. Community Relations
a. Represents the hospital in a positive manner in the hospital and within the community.
b. Participates in the marketing effort through internal and external activities.
